It is a nummular eczema whose diagnosis is clinical.
The treatment is local. Emollients and topical corticosteroids are the standard treatment.
Nummular eczema
Emollients participate in the epidermal restoration at the site of healing lesions and fight against itching of dry lesions. A glycerol/petroleum jelly/paraffin cream preparation can be used for up to two applications per day.
At Mrs. Phiso, you can opt for high-activity topical corticosteroids (betamethasone 0.05% cream). The amount applied must be sufficient as well as the processing time. Evening application may be recommended until clinical remission. Once this has been achieved, it is prudent to carry out a therapeutic decrease to avoid the rebound effect: switch to one application every two days for about ten days, then/or to two applications per week for a month.
In any case, you should take over with an emollient cream to moisturize the skin.
It is also necessary to insist on adjuvant measures : short bath (5 minutes) daily or shower at a warm temperature (27-30°C), use of soap-free bars or gels, favor cotton clothing, avoid wool clothing and synthetic fabrics next to the skin, wash clothing with phosphate-free detergent and fabric softener (to fight against dryness) and avoid any contact with tobacco.
Oral H1 antihistamine therapy is sometimes used if pruritus is severe, but there is no evidence of its effectiveness. In case of itching, applying cold water or a damp cloth sometimes relieves.
Allergological explorations are useless in most cases.
The use of antiseptics is based more on notions of habit than on proven effectiveness.
