{"id":10529,"date":"2024-11-01T12:35:40","date_gmt":"2024-11-01T11:35:40","guid":{"rendered":"https:\/\/biolaser.fr\/?p=10529"},"modified":"2024-10-26T13:21:27","modified_gmt":"2024-10-26T11:21:27","slug":"bowens-disease-mb","status":"publish","type":"post","link":"https:\/\/biolaser.fr\/en\/bowens-disease-mb\/","title":{"rendered":"Bowen&#8217;s disease (MB)"},"content":{"rendered":"\n<div class=\"wp-block-columns BlockVideo is-layout-flex wp-container-core-columns-is-layout-9d6595d7 wp-block-columns-is-layout-flex\">\n<div class=\"wp-block-column is-layout-flow wp-block-column-is-layout-flow\">\n<p><strong>BD is an intraepithelial squamous cell carcinoma (in situ).<\/strong> The population most concerned is the <strong>7th decade and the female sex.<\/strong> The genital forms are strongly linked to human papillomaviruses (HPV).<\/p>\n\n\n\n<p>From an anatomopathological point of view, the epidermis is disorganized and made up of atypical keratinocytes throughout its thickness, but which, by definition, do not cross the basement membrane.<\/p>\n\n\n\n<p>Bowen&#8217;s disease <strong>can progress in 3 to 5% of cases to invasive carcinoma<\/strong>, in very variable time frames. And it is readily associated with other cutaneous carcinomas (basal or squamous cell). Hence the importance of performing a complete skin examination to look for other cancerous or precancerous lesions.  <br\/><br\/><\/p>\n<\/div>\n\n\n\n<div class=\"wp-block-column is-vertically-aligned-center is-layout-flow wp-block-column-is-layout-flow\">\n<figure class=\"wp-block-image size-large\"><img fetchpriority=\"high\" decoding=\"async\" width=\"1024\" height=\"742\" src=\"https:\/\/biolaser.fr\/wp-content\/uploads\/2024\/09\/img-1-1024x742.jpg\" alt=\"Bowen's disease\" class=\"wp-image-10086\" srcset=\"https:\/\/biolaser.fr\/wp-content\/uploads\/2024\/09\/img-1-1024x742.jpg 1024w, https:\/\/biolaser.fr\/wp-content\/uploads\/2024\/09\/img-1-300x217.jpg 300w, https:\/\/biolaser.fr\/wp-content\/uploads\/2024\/09\/img-1-768x557.jpg 768w, https:\/\/biolaser.fr\/wp-content\/uploads\/2024\/09\/img-1-1536x1113.jpg 1536w, https:\/\/biolaser.fr\/wp-content\/uploads\/2024\/09\/img-1-2048x1484.jpg 2048w\" sizes=\"(max-width: 1024px) 100vw, 1024px\" \/><\/figure>\n<\/div>\n<\/div>\n\n<div class=\"wp-block-columns BlockTexte is-layout-flex wp-container-core-columns-is-layout-9d6595d7 wp-block-columns-is-layout-flex\">\n<div class=\"wp-block-column is-layout-flow wp-block-column-is-layout-flow\">\n<h3 class=\"wp-block-heading has-text-align-center\">Diagnosis<\/h3>\n\n\n\n<p>Talking about <strong>Bowen&#8217;s disease<\/strong> (squamous cell carcinoma in situ) on a clinical basis can be difficult.<br\/><br\/>In the presence of this single lesion located in a photo\u2010exposed area, with an erythematocrustous disc appearance, well limited, with a scalloped border and a very slow evolution in an elderly patient, the diagnosis of Bowen&#8217;s disease should be cited as a first line of treatment.<\/p>\n\n\n\n<p>Diagnostic confirmation is obtained by <strong>biopsy<\/strong>. Some general practitioners perform this procedure. Otherwise, the patient must be referred to a dermatologist who will take care of the biopsy and who will then organize the treatment (in case of diagnostic confirmation).  <br\/><br\/><\/p>\n<\/div>\n<\/div>\n\n<div class=\"wp-block-columns BlockTexte is-layout-flex wp-container-core-columns-is-layout-9d6595d7 wp-block-columns-is-layout-flex\">\n<div class=\"wp-block-column is-layout-flow wp-block-column-is-layout-flow\">\n<h3 class=\"wp-block-heading has-text-align-center\">Treatment of Bowen&#8217;s disease and carcinomas in situ<\/h3>\n\n\n\n<ol class=\"wp-block-list\">\n<li><strong>Small lesion: 2 options:<br\/><\/strong>&#8211; excision with histopathological control,<br\/>&#8211; &#8220;aggressive&#8221; cryotherapy (optimal freezing time: 10, 20, 40 sec?), under 3 conditions:<\/li>\n<\/ol>\n\n\n\n<ul class=\"wp-block-list\">\n<li>avoid treating legs and areas of precarious trophicity,<\/li>\n\n\n\n<li>verification of the diagnosis by biopsy,<\/li>\n\n\n\n<li>possibility of subsequent follow-up.<\/li>\n<\/ul>\n\n\n\n<p><strong>2. If surgery and cryotherapy seem difficult (extensive or multiple lesions or poorly healing areas):<\/strong> local chemotherapy: 5-FU, PDT, imiquimod with 2 conditions: diagnostic verification by prior biopsy and post-treatment monitoring.<\/p>\n<\/div>\n<\/div>\n\n<p>3. You should take advantage of the time of this consultation to insist on <strong>photoprotection measures<\/strong> with your patient. The recommendations of the American Academy of Dermatology are: <\/p>\n\n<ul class=\"wp-block-list\">\n<li>Seeking shade;<\/li>\n\n\n\n<li>Avoid exposure from 10 a.m. to 4 p.m.;<\/li>\n\n\n\n<li>Wear a sunscreen with an SPF (&#8220;sunburn protection factor&#8221; = sun protection factor) of at least 15, 15 to 30 minutes before exposure and reapply every 2 hours outdoors even on cloudy days;<\/li>\n\n\n\n<li>Wearing light, long, tightly knitted clothes, wide hat;<\/li>\n\n\n\n<li>No tanning outdoors or in a UV booth.<\/li>\n<\/ul>\n\n<p>4.
